State of Puerto Rico Eligibility Requirements
ELIGIBILITY FOR MEMEBERS:
Honorably discharged Veterans of the United States Armed Forces, spouses of Veterans, widows of Veterans and “Gold Star Parents” of a deceased Veteran in combat.
Application Process
The application is completed through the facility in conjunction with the Veterans Advocate’s office. The application includes information regarding military service, personal history, financial status and medical history. If the veteran has been recently hospitalized, copies of his/her current medical status should accompany the application.
Structure & Governing board
La Casa del Veterano was finished in 1995 in Juana Diaz on 15 acres donated by the County of Juana Diaz with an inversion of 14 million dollars. The home is currently under the administration of Ambassador of Veteran Services of PR, LLC. (AVS). AVS is a Company certified by the Department of State and consigned to the Veterans Advocate Office to operate La Casa del Veterano located at Road 592 KM. 5.6 Bo. Amuelas # 115, Juana Diaz, PR 00795. AVS started to operate La Casa del Veterano since June 1, 2014. The facility offers extended health care services for: Veterans of the United States Armed Forces; Spouses of Veterans; Widows of Veterans; “Gold Star Parents” parents of a deceased Veteran in combat.
Services Provided
Long-term nursing and domiciliary care with complete medical and supporting ancillary services.
