Application Process
An application can be obtained through our Admissions Office or can be downloaded from the LISVH website. A veteran can apply for admission to the Home directly from the community, following a hospital stay, or through a transfer from another skilled nursing facility. Completed applications are reviewed by an Admissions Committee comprised of members of the Home’s clinical team. The Home accepts ALL veterans regardless of the period during which they served. Our Adult Day Care program is open both to veterans and their dependents.
Structure & Governing board
Operates under the auspices of the Health Sciences Center of the Stony Brook University . The Home serves as a clinical training programs for physicians, nurses and other allied health professionals. The Governor appoints a Veterans Advisory Board that operates in an advisory capacity to the Home’s Executive Director. Nine veterans from local Veterans Service Organizations serve as members of this Advisory Board.
Services Provided
A comprehensive range of services to address the medical, nursing, rehabilitation, psychosocial, recreational, spiritual and personal needs of the residents. The Home includes two specialized units, an Alzheimer’s/Dementia unit and a Respiratory unit. The Home also operates a medical model Adult Day Health Care program.
