ELIGIBILITY FOR MEMEBERS:

Application Process
Applicants apply for admission on the form provided by ASVH-P. The form requests demographic and financial data. Applicants must submit a copy of the discharge document or equivalent separation-of-service document. Applicants must submit a certified copy of the applicant’s marriage certificate if the applicant is a spouse or surviving spouse. A certified copy of the veteran’s death certificate must be submitted if the applicant is a surviving spouse. Admissions committee determines final approval of applicant after veteran’s status is verified by a State Veteran’s Service Officer.
Structure & Governing board
The ASVH-P Administrator receives direction from the director of the Arizona Department of Veterans Services. Facility is operated in accordance with the Arizona Administrative Code, the Arizona Department of Health Services Office of Health Care Licensure, and the Department of Veterans Affairs.
Services Provided
Skilled nursing care 24 hours a day. Medicare and sub-acute unit. Alzheimer’s unit. Speech, physical, occupational and respiratory therapy. Hospice services. Respite care, therapeutic meals, recreational therapy and social services. Over-the-counter medications provided at no charge. Personal hygiene items provided at no charge.
